For your convenience, you may complete the OPS registration form on-line by selecting the Microsoft Word icon below. Simply complete the form, print it out, sign it, and bring it with you to registration. If your child will be attending Owensboro High School, please complete the High School Enrollment form. For all other schools, please select the P – 8 Enrollment form.
